Camp Foster ID Card Services (Appointment and Walk-in)

Booking an appointment the same day must be within 5 hours of the next available time slot. Promotion CACs, Expiring CACs, Expiring USIDs (Dependent/Retiree ID’s), New Employee CAC, Email/Credential updates, DEERS updates, 100% Disable Vet ID's, DD Form 1172-2, CAC Blocked/Pin Reset, Lost or stolen CAC, Newborn Enrollments, and Agent Letters.

PLEASE NOTE:

The ID card office can NOT manually update or add any of the following information: Promotions, Active duty to Retiree status, Extensions, Reservist on active orders, Active duty to Reserve status.

For uniformed services personnel and DoD civilians, all submissions to DEERS must be made electronically via an authorized data source feed (e.g., Civilian Personnel Management Service). Data source feeds for additional population categories shall be approved and incorporated by the Office of the USD(P&R) (OUSD(P&R)) as they become available.

Due to monthly promotions, a high volume of customer on the first of the month is expected. It is strongly recommended to schedule an appointment. Appointments will take precedence.

Camp Foster ID Card Services (Walk-Ins)

The ID Card Center on Camp Foster will take walk-ins for the following reasons:  CAC Blocked/Pin Reset, Lost or stolen CAC, Newborn Enrollments, and Agent Letters 

ID Card Appointment Website:

Appointments can be made at https://idco.dmdc.osd.mil/idco/ > Select make appointment > Enter location (Chatan) > Select 50 miles in radius search > Select search > Scroll down to select desired DEER’s location > Select schedule an appointment > Search calendar for available appointment slots > Click on date and select book appointment with available time slots > Fill in required information and save > Confirmation email will be sent to email address provided. Please book one appointment per transaction to ensure the appropriate amount of time is available.

Overseas Base Privileges:

If the Sponsor and dependents are authorized base privileges overseas (Commissary, Exchange, MWR) please bring the following documentation: DD 1172-2 stating base privileges are authorized or the letter of employment stating base privileges are authorized. ***Contractors*** will need to provide the letter of authorization (SPOT LOA) and assigned to overseas for longer than 365 days.

Identification Requirements:

ALL Sponsors, Spouses and Dependent Children ages 18 years and older must provide two unexpired forms of ID to receive a new ID card. One must be a unexpired State/Government issued Photo ID.

Acceptable Identification Examples:

Primary: Military ID, Driver's License, State ID, Passport. Secondary: A second Primary ID, Social Security Card, Voter's Registration Card, Birth Certificate, or Veterans Health Identification Card (VHIC). All must be unexpired in original form.

Foreign Documentation:

All foreign documents MUST have a certified English translation (if applicable) and an Apostille or Consular certification. Contact the document issuing facility to obtain authentication certification.

USID Card Online Renewal Services

Sponsors may request Uniformed Service ID (USID) Card renewal online, which if approved, are mailed directly to the card recipient; no in-person appointments necessary.

Expiring CACs

CACs will be renewed no earlier than 90 days out from expiration date. Emergency situations will be taken into consideration and approved by ID card supervisor.  

Expiring USIDs (Dependent/Retiree IDs)

USIDs will be renewed no earlier than 90 days out from expiration date. Emergency situations will be taken into consideration and approved by ID card supervisor. 

New Employee/Renewal CAC (Civilian/NAF)

Documents Required: Completed DD Form 1172-2 signed by the Civilian Personnel Office, Letter of Employment and two valid forms of identification.

New Employee/Renewal CAC (Contractor)

The Trusted Agent (TA) for the contractor’s sponsoring organization must add the contractor to DEERS through the Trusted Associate Sponsorship System (TASS) before a CAC can be issued. Documents Required: Letter of Authorization, Completed DD Form 1172-2 signed by authorized signatory and two valid forms of identification. 

Frocking CACs:

Please provide a letter authorization or web orders for verification purposes.

Initial Retiree IDs:

For uniformed services personnel and DoD civilians, ALL submissions to DEERS must be made electronically via an authorized data source feed (e.g., Civilian Personnel Management Service). Data source feeds for additional population categories shall be approved and incorporated by the Office of the USD(P&R) (OUSD(P&R)) as they become available.

“CAC” Blocked vs “Account” disabled:

Report to the nearest ID card center for all “CAC” blocked or locked error messages. All CAC unlocks and CAC pin resets need to be conducted in-person as verification of recipient's fingerprints are required.

Contact your local IT support for all “Account” disabled or suspended error messages. Users with @USMC.mil email addresses can call DSN 117 for “Account” related issues

Lost or Stolen CAC:

All personnel (Military/Civilian) whose CACs have been lost or stolen must bring a Minor Offense Report from the Provost Marshall Office or Memorandum for Record from sponsoring agency signed by a supervisor reporting it was or lost/stolen.

Lost or Stolen Dependent/Retiree ID:

PMO report not required. Sponsor must be present to sign 1172-2 for new ID card. Two valid forms of ID’s are also required for dependents 18 years of age and older.

ID CARD CERTIFICATE RECOVERY 

Follow the below steps at https://ara-5.csd.disa.mil

1.   User needs to navigate to DOD Automatic Key Recovery webpage and download previous encryption certificate.

      a.   Certificate should match time period of when the email was sent.

      b.   Download File to computer

      c.   Save the One-Time Password onto the users workstation temporarily (will need to enter later).

2.  Install downloaded Encryption Certificate.

     a.  Open Outlook and navigate to: File > Options > Trust Center > Trust Center Settings > Email Security.

     b.  Click on Import/Export (next to Publish to GAL).

     c.   Browse to file location and select appropriate file.

     d.   Enter in One Time Password a.  Open Outlook and navigate to: File > Options > Trust Center > Trust Center Settings > Email Security.

3.  Attempt to access encrypted email.

GAL Update:

Users can update their GAL and contact information at the ID Card Office online website https://idco.dmdc.osd.mil/idco/ under the MY Profile tab.

Secure Area Access:

All recipients that receive a new CAC need to contact their respective Security Manager or Provost Marshall Office to regain access to secure areas they were previously authorized. Secure access does not transfer over to the new issued CAC.

Updating DEERS Record:

Sponsor must be present, or a valid Power of Attorney will be required for all record updates. Valid supporting documentation must also be provided.

Adding Spouse to DEERS:

When adding a spouse to DEERS, the sponsor must bring the following documents for their spouse: Marriage Certificate, birth certificate, social security card and a valid photo ID. All documents must be in original form or certified true copy.

Adding children to DEERS:

Sponsor or Sponsors (Dual Military) must be present. Required Documentation: Marriage Certificate/License, Birth Certificate or Proof of Birth, Social Security card (If Available). Stepchild: Marriage Certificate required. Illegitimate child: Voluntary Acknowledgement of Paternity or Judicial Determination of Paternity is required.

Full Time College Students:

Sponsor must present a letter from the school's registrar office stating currently enrolled or registered as a full-time student. School must be an accredited college, and student must be in pursuit of an associate degree or higher. Sponsors must also certify on the 1172-2 form that they are providing over 50% to support the child.

Dependents without Sponsor Present:

All Dependents without a Sponsor present must have a valid DD Form 1172-2 pre-printed by a DEERS station that has been signed by the Sponsor and a Verifying Official ahead of time. These signed 1172-2 are valid 90 days from signature date. Power of Attorney accepted but must state DEER’s or Military ID card related transactions.

 Printer Service Not Available:

We do not offer any printing services; all members must come prepared with physical copies of any required documents. We also cannot receive emails with said documents, all documents must be printed before coming to the customer support section for a CAC/Dependent ID card.

100% Disabled American Veteran ID:

 The veteran will provide a copy of their DD-214 & the final 100% disability award letter provided by the VA. 

Navy Personnel Alternate Tokens Request:

Due to limitations with Active Directory, the Common Access Card (CAC) can only be used for network logon to one account. Appointments: All applicants, including children, must be present. Walk-ins limited to 5–10 applications per session.

Payment: Only money orders or cashier’s checks accepted. Available at Community Bank or Post Office. See "Payments" in the menu.

DoD Local Hires: Generally, locally hired US citizens use their regular-fee (blue tourist) passports when performing official USG travel (Ref. FCG). Verify eligibility with local CHRO.

Passport Photos: Requirements: Color 2"x 2" Passport Photo.

1.  PASSPORT FEES.  Passport fees will increase by $20 for all types of passport applications beginning on December 27, 2021.  A complete schedule of passport fees may be found here:  https://jp.usembassy.gov/services/fees/ 

 

2.  SPECIAL ISSUANCE PASSPORT (NO-FEE PASSPORT) REQUIREMENT FOR ENTRY INTO JAPAN UNDER OFFICIAL ORDERS

ATTENTION:  Per a recent update to the Foreign Clearance Guide, DoD Civilians and Eligible Family Members returning to Japan on official travel must have either a Special Issuance Passport (SIP) or, as a temporary measure, a memorandum from a DoD Passport Agent stating the SIP application was submitted prior to departure from the point of origin. The memorandum must identify each applicant, date of submission, country of travel for PCS/TDY/TAD and DoD VPAS ID. For a limited time, boarding a flight while on official travel status with a Regular (blue tourist) Passport is permissible ONLY with such a memo, which is to be presented to boarding officials, upon request.

NOTE: This temporary exception, i.e., allowing official travel on a Regular Passport with memorandum (in lieu of a SIP), will expire on 31 March 2022. Effective 1 October 2021, official travel must be conducted with a SIP. See I.A., below for additional requirements. (additional information is provided in the Foreign Clearance Guide).


Therefore, DoD Civilians and Eligible Family Members who plan to conduct international travel under orders, who require re-entry into Japan and do not possess a no-fee passport, must apply for a passport and request a memo from the DoD Passport Agency processing their passport application.
