  GROUP ACCESS PASS (GAP) More than 5 visitors/Escorted Only
  
  GAP FORM – Hand writing is not acceptable.
  
  PARENT PERMISSION SHEET (JSG 1005)
  
  Require the JSG Form for guest under 18yrs who is not accompanied by their parents.
  
  After complete the form, please send it to the following address:

  Please allow 3-5 business days to process the request
- Temporary Pass
  TEMPORARY PASS (Up to 90 days)
  
  Letter of Request (LOR)
  
  Background Check (SECNAV 5512.1)
  
  Complete all checklist and take it to Bldg. 496/PMO HQ/645-3886.
  
  *If a visitor is a U.S. citizen, make sure one of the box for #37 is checked by PMO.
  
  - Proof of citizenship (Copy of passport or Koseki-shohon)
  
  - Copy of Driver's license or my number card for a Japanese.
  
  - Copy of Resident card if applicant is a foreigner.
  
  Please allow 3-5 business days to process the request
- Semi-Annual Pass
  SEMI-ANNUAL PASS (Up to 3 years for MLC and IHA / 2 years for others)
  
  Letter of Request (LOR)
  
  Background Check (SECNAV 5512.1)
  
  Base Pass Application Form
  
  Complete all checklist’s and take it to Bldg. 496/PMO HQ/645-3886.
  
  *If a visitor is a U.S. citizen, make sure one of the box for #37 is checked by PMO.

Camp Foster Pass Office issues base access passes for contractors, government employees and visitors. All access applicants must adhere to Marine Corps policy and meet policy requirements.
